چکیده

Amyloid-like immunoglobulin M (IgM) deposition neuropathy associated with Waldenström macroglobulinemia is a rare phenotype of IgM-related neuropathy. The prominent clinical features are painful sensory-dominant neuropathy followed by distal motor weakness and atrophy over a long clinical course that are refractory to immunosuppressive treatment in many patients. Herein, we report a case of amyloid-like IgM deposition neuropathy that was successfully treated with rituximab (RTX).

Successful Diflunisal Desensitization in a Transthyretin Amyloidosis Patient with NSAID Allergy: A Case Report

Introduction: Amyloid diseases have been known to be hereditary, including transthyretin (TTR) amyloidosis where subunit protein mutations may occur in genes for TTR leading to the deposition of fibrils (low molecular weight subunits (5 to 25 kD) of proteins) in extracellular tissues. By reducing the formation of TTR amyloid, diflunisal, a nonsteroidal anti-inflammatory drug (NSAID), has shown ...

Familial Amyloid Polyneuropathy Type IV (FINNISH) with Rapid Clinical Progression in an Iranian Woman: A Case Report

Familial amyloid polyneuropathy (FAP) type IV (FINNISH) is a rare clinical entity with challenging neuropathy and cosmetic deficits. Amyloidosis can affect peripheral sensory, motor, or autonomic nerves. Nerve lesions are induced by deposits of amyloid fibrils and treatment approaches for neuropathy are challenging. Involvement of cranial nerves and atrophy in facial muscles is a real concern i...

Neurofascin antibodies in autoimmune, genetic, and idiopathic neuropathies

OBJECTIVE To measure the frequency, persistence, isoform specificity, and clinical correlates of neurofascin antibodies in patients with peripheral neuropathies. METHODS We studied cohorts of patients with Guillain-Barre syndrome (GBS) or chronic inflammatory demyelinating polyneuropathy (CIDP) (n = 59), genetic neuropathy (n = 111), and idiopathic neuropathy (n = 43) for immunoglobulin (Ig) ...

Cutaneous macroglobulinosis: a report of 2 cases.

BACKGROUND Specific cutaneous lesions of Waldenström macroglobulinemia are rare and include neoplastic cell infiltrates, IgM bullous disease, and so-called IgM-storage papules, which characterize cutaneous macroglobulinosis (CM). OBSERVATIONS We report 2 patients with CM. In patient 1, CM started as small papules, as reported in most of the previously published case studies of CM. In patient ...

Immunotherapy-based regimen in anti-MAG neuropathy: results in 45 patients.

The prevalence of neuropathy in patients with immunoglobulin M (IgM) monoclonal gammopathies ranges from 5% to 31%. The most frequent neuropathy is associated with monoclonal IgM reacting with myelinassociated glycoprotein (MAG) and is usually a chronic demyelinating disorder that typically presents with progressive ataxia and painful paresthesias. The clinical features of neuropathies associat...
